Courses of Study Forms & Faculty Guidance Sheet (updated April 2022)

 
Faculty Guidance Sheet – this document guides faculty in the process of submitting curricular proposals on the appropriate forms to the GSE Courses of Study (COS) Committee.

 

FORMS
Minor Modifications Proposal Form: This form is used to propose minor modifications to an existing course or program. Examples of minor modifications include changing the course name, scheduling, instructional delivery format, or prerequisites.

 

New Course Proposal Form: This form is appropriate for proposing a new course.

 

Course Change Proposal Form: This form is appropriate for proposing significant or multiple changes to an existing course.

 

New Program Proposal Form: Requires Pre-Approval from the Chancellor-Provost Office before submission to COS. Complete a Letter of Intent (LOI)

 

Program Change Proposal Form: This form is appropriate for proposing significant or multiple changes to an existing program. Refer to the Program Checklist to see if any of the (12) Program Checklist questions are answered “yes”. If so, a Letter of Intent (LOI) is required.
